"Nope" pt wearing jeans, Forgetting to check the patient in all the way, "My doctor ordered my scan this morning/afternoon and I haven't been called to schedule it yet.", "Has somebody ever answered yes to these questions?", Pt creatinine too high and Pt at window for a stat scan and provider's office never called.
