Improve Mob Formations, more variety not just packs
Change Auras so they only give a set amount of their bonuses to allies.
Revisit Ward and make it an interesting defense layer
Fix Life Leech/Recovery
Commit to Flask Changes and remove all dependency on flasks for defences (Buff other sources)
Continue with league mechanics giving you the choice of when to engage with them (eg. TotA Silver Coins)
Give Two Handed weapons some unique implicit stats (More Base AoE etc)
Rare Mods based on the mob type (Zombies get chaos auras or revival or dropping tar as rare mods)
Modernize Cast on Crit, give skills proc coefficients to de-emphasize all CoC builds only using skills like Cyclone
Make Minions who are controlled by your Character Actions (They Fire Arrows when you do etc)
Alleviate Socket Pressure with an interesting mechanic or innovation
Bring PoE2 Stun system to PoE1
Give the option for Energy Shield to actually be a defensive layer and not just Blue Life
Evolve the Transfigured Gems system by just giving Skill Gems their own Passive Trees

Rework Dual Wielding and add mods that incentivize using two weapons
Balance Baseline Build Viability
Give more interesting soul-bound rewards like Sanctified Relics
Innovate on visually communicating enemy power level (Reading Mods isn't optimal)
Rework Summoning and Minions (Employ some of PoE2 changes if possible)
Add some form of Spell Resistance to left side of the tree as a Str/Int version of Spell Suppression
Rebalance Auras make the stats they provide more accessible but Aura Effect is stronger when invested in
Add a new socket type that can be "flipped" using a hotkey if it has a support gem socketed in it
Make Drops more valuable by having more mods that only spawn on dropped items
