  1. Gather supplies prior to starting test
  2. Update patient reported medication profile with warfarin dosing
  3. Clean meter with 70% isopropyl alcohol wipe
  4. Lancets to be disposed in stable container with lid.
  5. Place sample on side of test strip
  6. Keep meter level when testing
  7. Results 4.0 or greater should have stat protime and INR via venipuncture
  8. Never wait more than 15 seconds after sticking the fingertip before applying the blood.
  9. Strip container should remain closed when not in use
  10. Make sure fingertip is thoroughly dry prior to lancing
  11. Use gloves when performing test
  12. Never touch any buttons while a test is in progress
  13. Never open a container of test strips or touch a test strip with wet hands or wet gloves
  14. Do not take coagucheck machine into isolation patient home
  15. Turn meter off when changing code chips
  16. Update care plan with new orders
  17. Never add more blood after the test has begun
  18. Clean patient finger with alcohol wipe
  19. Blood should be collected on the side of the finger
  20. Never remove or insert the code chip while the meter is performing a test.
  21. Turn meter off when changing the batteries
  22. Ensure there is a visit on the schedule for when the next INR due
  23. Never perform a test with a drop of blood from a previous fingerstick.
  24. Never store the meter in damp or humid conditions (greater than 85% humidity).
